Medical Records:

All patients seen at IGA/ASC have a medical record. Patients can obtain copies of records simply by signing consent for release of information. There is a retrieval fee of $5.00 and a charge of $1.00 per page for pages 1-25, $0.50 per page for pages 26-500, and 0.25$ per page for 501 pages and above (in compliance with The Louisiana Health Care Information Act- R.S. 40:1299.96). When copies of records are needed, please call at least a week in advance to allow for preparation. Someone other than the patient may pick up the records if the patient has given specific consent. If records are needed to continue care by another physician, they can be sent directly to the physician at no charge.
