Payment Methods:
Charges are payable at the time treatment or service
is given. Regardless of your medical insurance coverage, our office
relies on you to settle your account. For your convenience, we offer
the following payment options:
1. Payment in full or payment of the portion your
insurance will not cover on the day service is provided.
2. Payment of balance in full upon receipt of your
statement.
3. Use of your VISA, DISCOVER or MASTERCARD.
Our business office will submit primary and secondary
insurance claims for you - subject to your having given us current
insurance information prior to the service being performed. You
will not receive a bill from us until your insurance has made payment.
However, if you make payment arrangements, you are expected to comply
with the terms of the agreement as stated.
If any other arrangements are needed, please talk
to our Patient Accounts Representative prior to receiving service.
You will notice all statements have a Birmingham,
Alabama address. Statements are processed locally in our office;
however, payments are to be remitted to the Birmingham address.
This is an outside company contracted to receive and process payments
for ease of deposit.
