Billing Information
for Procedures Performed at Acadiana Surgery Center, Inc.
We are happy to file claims with your insurance company for services
provided. To ensure that your claims are paid appropriately, and to
reduce unnecessary cost to you, please be sure to provide the receptionist
with your most current insurance information. Include all secondary/supplemental
policies.
Sometime before your procedure, we will call you with an estimate
of your portion of the bill. Unfortunately, we are only able to
estimate your portion of the bill based on the information
provided by your insurance, and based on the procedure ordered.
Sometimes, during the course of a procedure, Dr. Stokes may find
it necessary to remove a polyp, or take a biopsy, etc. This may
increase the cost of your procedure. Therefore, the amounts quoted
by our office prior to your procedure are only estimates.
If our office has not contacted you by the day before your procedure,
and you do not have 100% coverage by your insurance company (or
supplement), please call our office to obtain these estimated costs.
When you come in for your procedure, the receptionist will collect
the amounts you have discussed on the pre-procedure phone call.
Due to the fact that you will be sedated for the procedure, these
amounts are collected before the procedure.
Iberia Gastroenterology Associates, Inc. and Acadiana Surgery Center,
Inc. are two separate corporations. You will receive a bill for
Dr. Stokes' fee and a facility fee for Acadiana Surgery Center,
Inc. This is not considered an office procedure; therefore, office
visit copays do not apply. If paying by check, please write separate
checks for Dr. Stokes and Acadiana Surgery Center, Inc.
You will not receive a bill for your procedure until after your
insurance company has paid. However, if you make payment arrangements
for your share of the bill, please send these payments monthly as
stated in the agreement.
